Pentagon research shows a U.S. intelligence analyst warned in 2019 that a former Iranian military site in Minab had been turned into an elementary school. The warning was logged in a database that was not linked to the official target‑selection system and never reached commanders.
According to Bloomberg, the school was hit during the initial U.S. strikes on Iran on 28 February 2026, killing about 120 children and nearly 200 other civilians.
The investigation is examining the use of outdated intelligence, lack of coordination between information systems, and whether required target‑validation steps were followed. The final report has been sent to CENTCOM command and remains under review.
